One Heart Camp 2026 Registration Form

Middle School AND High School happening simultaneously

(same theme and education breakdown; some activities may potentially be combined as a whole camp)

Dates: Sunday, June 7th to Saturday, June 13th

Location: Camp Seely in Crestline, CA

For who: Middle School (finished 5th grade, incoming 6th graders Fall 2026) AND High School

Fundraising and Scholarships:

We encourage participants and youth groups to fundraise (car wash, bake sales, GoFundMe, etc.) and support your youth to attend. Youth Pastors are aware and will help in coordinating fundraising.
Kitchen Staff help is needed!! - 1 full scholarship is available per parent kitchen staff. To apply to be a kitchen staff please fill out this form here. Fill it out and wait for our response BEFORE submitting your child's participation form.

Price:

Early Bird Price: $425 per child (until 11:59pm PST Sunday, April 19th)
Regular Price: $450 per child (until 11:59pm PST on Sunday, May 17th)
Late Registration from May 18th: $475
Walk In: $525
Refund will be half after May 18th
$15 off for siblings

Flying in? --> We will ONLY coordinate transportation to and from the airport for those flying in to Ontario Airport in San Bernardino County, CA. $20 transportation. (full details in the flight form: flight info submission
Flights: Arrive ON Sunday, June 7th before 2pm. Depart Saturday, June 13th after 3PM. If you arrive a day early or depart a day after please coordinator your own lodging and transportation with families in LA.

If I fail to bring the physical copy of the waivers, I understand that I won't be allowed to participate and attend camp. We need the physical copy for liability.

Cash and checks are preferred. All payment should be completed prior to the deadlines (see beginning of form).

After June 15th, cancelations will be refunded by half and not full amount.

Code of Conduct Agreement

Please read this Code of Conduct with your child/youth to acknowledge your/his/her acceptance of, and willingness to adhere to the mentioned standards. If for any reason your child/youth is asked to leave the workshop, you/your family will be required to pay any costs incurred in your return home.

As a participant in One Heart Camp - West Region Middle & High School workshop, I will abide by all the following conditions and requirements:

1. I will not smoke, drink alcoholic beverages, or take illegal drugs, or have any of these items in my possession. This behavior is grounds for immediate and unconditional expulsion from the workshop.

2. I will not deface any property or site, nor will I support or conceal such actions of others. I understand that I will be asked to leave the workshop immediately if I behave in this manner.

3. I will not engage is promiscuous or flirtatious behavior and I will dress according to the workshop dress code standards.

4. I will fully participate in all programs and activities unless the workshop nurse indicates that I am too injured or sick to participate.

5. I will respect and support all workshop site rules and regulations and encourage others to do the same.

6. I will fully participate with my assigned group, listen to my group leaders and workshop staff and I will always obtain my leader’s permission in order to leave team activities.

7. I will respect and support all people at the workshop site and I will not engage in or support any activity that may put others in physical, emotional, or spiritual danger.

8. I will make every effort to conduct myself in a dignified manner befitting a son or daughter of God.

If for any reason your child/youth is asked to leave the workshop, you/your family will be required to pay any costs incurred in your return home.

I have gone over this with my child/youth so they understand the guidelines and standards at One Heart Camp.
